Information we collect
We only collect what we need to respond to you and provide our services.
- Contact details you give us, such as your name, email address, phone number and address.
- Details about the service you are requesting, entered in quote, booking or contact forms.
- Technical data such as browser type, device and pages visited, collected through cookies you allow.
- Records of our communications with you, including calls, emails and messages.
How we use your information
We use personal information to:
- Respond to enquiries and prepare quotes or appointments.
- Deliver, schedule and follow up on the services you request.
- Send service reminders and, where you have agreed, occasional offers.

Sharing

HIPAA notice of privacy practices
QuickCare Urgent Care is a covered entity under the Health Insurance Portability and Accountability Act. This summary describes how medical information about you may be used and disclosed, and how you can get access to it. The full notice is available at the front desk of every clinic and is provided to you at your first visit.
We use and disclose protected health information without your authorisation for treatment, for payment, and for health care operations such as quality review and staff training. We may also disclose it where the law requires: communicable disease reporting to the Oklahoma State Department of Health, suspected abuse or neglect, workers' compensation claims, public health surveillance, and valid judicial or administrative orders.
- Right of access: you may inspect and obtain a copy of your record, normally within 30 days of a written request.
- Right to amend: you may ask us to correct information you believe is inaccurate or incomplete.
- Right to an accounting of disclosures: you may request a list of certain disclosures we have made.
- Right to request restrictions: you may ask us to limit how we use or disclose your information. We must agree to a restriction on disclosure to a health plan when you have paid for that item or service in full yourself.
- Right to confidential communications: you may ask us to contact you at a particular address or number.
- Right to be notified of a breach of unsecured protected health information.
- Right to a paper copy of the full notice, even if you agreed to receive it electronically.
Occupational health and employer disclosures
Where a visit is arranged and paid for by an employer, the results we release are limited to what the employer is entitled to receive, and go only to the authorised senders named on that account. Drug and alcohol testing results under 49 CFR Part 40 are released to the designated employer representative or the medical review officer, never to a supervisor who telephones the clinic.
For a respirator clearance under OSHA 1910.134, the employer receives the clearance conclusion only. The underlying medical questionnaire remains confidential between you and the examining clinician.
Complaints
If you believe your privacy rights have been violated, you may complain to us at the contact details above or to the Secretary of the United States Department of Health and Human Services. You will not be penalised or retaliated against for filing a complaint.
